2016-17 Choice Programs Application

Online Choice Application for the 2016-17 School Year

The deadline to apply for a Choice program was Friday, January 29, 2016. Late applications will be accepted but must be submitted in paper form.

Completion of a late application does not guarantee admission and may not be considered in the 2016-17 Choice lottery. Falsification of information on this application could lead to ineligibility for Choice Programs. Students applying for kindergarten must be 5 years old by September 1, 2016. Parent/guardian must be a resident of Palm Beach County when application is submitted. For more information, call (561) 434-8755.

Applications may only be submitted by parents/guardians. Student submitted applications cannot be processed.

Reminder: Only one application per student may be submitted. No changes are allowed after submission of a hard copy application.

Eligibility/Ineligibility of Applicants

There are no pre-k, elementary or middle school eligibility criteria except the following:

  • Bak MSOA – Passing audition score
  • Lake Worth MS and Roosevelt MS - Biomedical Sciences (FAU College of Medicine) – 3.0 GPA and passing score on an internal assessment conducted by FAU College of Medicine
  • Okeeheelee MS – Passing score on Spanish assessment test
  • The Conservatory School @ North Palm Beach (grades 6-8) - Instrumental Music Jury Assessment

However, students scoring Level 1 or Level 2 in Reading and/or Math on the state FCAT assessment may not be eligible to take the Choice program courses due to state requirements for remediation/intensive courses in Reading and Math for Level 1 and some Level 2 students.

The eligibility requirements for High School Choice programs are based primarily on GPA. GPA for incoming 8th graders only, is based on the average of all classes in the 4th quarter grade 7, all classes 1st quarter grade 8 and all classes 2nd quarter grade 8. For students in grades 9-11, the GPA listed at the time of eligibility determination from student TERMS will be used.

Deadline Date: Friday, December 4, 2015 for Bak MSOA, The Conservatory School @ North Palm Beach (Symphony Orchestra - grades 6-8), Boynton Beach High School Arts programs (Dance, Digital Media, Music-Band, Music-Keyboard/Piano, Music-Vocal, Theatre, and Visual Arts), and A.W. Dreyfoos Jr. School of the Arts and Friday, January 29, 2016 for all other schools.

Hard copy applications may be submitted; however, any application received after 4:00 PM on January 29, 2016 is considered late. Late applications accepted after January 29, 2016 are NOT included in the March 2016 Selection Lottery and may be entered only after all applicants in the wait pool have been assigned through random selection lottery.

The application must be printed, signed and mailed or delivered in person to The Department of Choice and Career Options located at 3308 Forest Hill Blvd., C-124, West Palm Beach, FL 33406. The original must be mailed or delivered with a postmark no later than December 4, 2015 Bak MSOA, The Conservatory School @ North Palm Beach (Symphony Orchestra - grades 6-8), Boynton Beach High School Arts programs (Dance, Digital Media, Music-Band, Music-Keyboard/Piano, Music-Vocal, Theatre, and Visual Arts), and A.W. Dreyfoos Jr. School of the Arts and Friday, January 29, 2016 for all other schools.

The Department of Choice and Career Options is not responsible for any application not received as a result of an error or delivery delay by the US Postal Service.
